"Welcome Home" Documents

"Welcome Home" Visit Request (07/2006)

The second draft of a letter, previously reviewed by MOPH Patriots and military personnel, to a State Adjutant General (AG) in command of National Guard forces. You can use this letter to ask an AG or other General Officer for permission to welcome home wounded and/or other troops at military facilities under his command. This letter can also be used for a Reserve Components or Active Duty Commander for similar requests.

MOPH "Welcome Home" Card (09/2008)

A special Thank You and Welcome Home card from the MOPH, four to a Letter size sheet of paper, for presentation to all returning OIF/OEF troops. It is recommended to print these cards on a heavy stock paper with 67, 90, or 110 pound weight for durability.

MOPH "Welcome Home" Tri-Fold Brochure (02/2009)

"Welcome Home" brochure of our MOPH Chapter 353 and Department of Virginia. It is designed for presentation to all OIF/OEF veterans. This brochure includes the National Suicide Prevention Lifeline telephone number and web links to information about the new Post 9/11 GI Bill and the right of veterans to hire a private attorney to challenge an adverse disability rating decision. Additional helpful information have also been added and/or updated. Please feel free to modify the MS Word documents and include your Chapter and/or Department contact information.

MOPH Life Membership Application (09/2008)

The MOPH Life membership application form – three to a Letter size sheet of paper. The name of the recruiting Patriot and Chapter number are left blank, for you to fill in for the recruiting credit. Special instructions for applicants are to print their personal information and provide one of the documents required for membership in the MOPH. For best results, print these forms on a heavy stock paper with at least 67 pound weight.
