"Welcome Home" Documents

"Welcome Home" Visit Request (07/2006)

The second draft of a letter, previously reviewed by MOPH Patriots and military personnel, to a State Adjutant General (AG) in command of National Guard forces. You can use this letter to ask an AG or other General Officer for permission to welcome home wounded and/or other troops at military facilities under his command. This letter can also be used for a Reserve Components or Active Duty Commander for similar requests.

Operation "Welcome Home" Project Tri-Fold Brochure (07/2007)

Operation "Welcome Home" Project brochure has no reference to the MOPH. It is suitable for presentation to non-battle injured (nbi) patients who may feel guilty or uncomfortable when being welcomed home by Purple Heart veterans. This brochure includes a web link to information about the right of veterans to hire a private attorney to challenge, as needed, an adverse disability rating decision. Several other helpful links have also been added. Please feel free to modify the MS Word documents and include contact information of your Chapter and/or Department.

MOPH "Welcome Home" Tri-Fold Brochure (07/2007)

"Welcome Home" brochure of our MOPH Chapter 353 and Department of Virginia. It is designed for presentation to combat-wounded veterans who has already received or are eligible to receive the Purple Heart Medal. This brochure includes a web link to information about the right of veterans to hire a private attorney to challenge, if needed, an adverse disability rating decision. Free MOPH Membership Application (05/2005)

The one-year free MOPH membership application form – four to a Letter size sheet of paper. The name of the recruiting Patriot and Chapter number are left blank, for you to fill in for the recruiting credit. Special instructions for applicants are to print their personal information and provide one of the documents required for membership in the MOPH. For best results, print these forms on a heavy stock paper with at least 67 pound weight.
